How much of GH *really* came from EGG's campaign?

I was just wondering how much of the GH campaign setting, as we know it, really came from EGG's own D&D campaign.

From my understanding, EGG's own campaign was based on a fantasy version of the Earth (like his Epic of Aerth material). When GH was designed for TSR, however, a whole new world was made but bits and pieces of EGG's campaign were added like: Greyhawk city, Greyhawk ruins, ToEE, etc.

Can anyone tell me how much of GH was created for publication, and how much originally belonged to EGG's home campaign?

Gygax writes up a good summary of Greyhawk's development in some of the Dragon Annuals, and there's also a good article in Horsemen of the Apocalypse published by Jolly Roger Games (that covers some of the same ground as the Dragon Annual articles, but goes beyond it).
