Has anyone else used - The Plague Years-
as seen in Best of GH by Ariel3 ?
I've run a continuous timeline over the last 24 years real time since 576cy and have never tried anything so broad as this. I am about to finish a modified Rabid Dawn from Dungeon #111. Hardby has been sacked and then reclaimed however the rest of the territory is still in danger including an all out assualt on Celene. The plague would fit well but I'm still hesitant to kill off that many npcs and pcs.
I could really use some input , any thoughts?

I am unfamiliar with The Plague Years. I'm curious to know if the title is literal. Are we talking an actual plague/disease that will kill off this large number of NPC's and PC's you speak of, or is it metaphorically speaking of something as a plague?
Yes, it is actually germ warfare for lack of a better desciption. A designer virus made by a group of elves and druids, it seems to not harm animals or magical beasts since these are not mentioned.

Well, from a DM's point of view, I would be very reluctant to kill off that many NPC's and PC's by plague or disease. You said you've been playing a timeline for 24 years in real time. If there are PC's that have been around for 24 years real time, I, without a doubt, could not bring myself to kill them off by of all things, a plague. They've been around too long to go out that way. If they are going to keel-over, I'd have to have them go out in a blaze of glory in battle or some such, not waste away by disease. That would cheapen the whole experience somehow to me. Plus, I would owe it to my players of that many years to have their heroic PC's for so long, die a hero's death.
From a player's point of view, if I had a PC that old in real time, and my DM killed him or her off by having them contract a plauge, I'd have to come across the table pencil first and stab him in the face. That's a lot of time and effort put into a PC for that long to die by plague. A lot of people might disagree, but I just find it a let down somehow, given the length of time we're talking about here. Not that these PC's should never be killed simply because they've been around a long time, but because of that time, I'm inclined to offer a bit of favoritism in the way they would finally bite the dust, and plague seems rather anticlimactic for heroes that have been around in real time for 24 years.
Well yes I agree, as both also , but on the other hand most of the high level PCs would be immune anyway and everyone not immune would get thier checks.
The main thing would be for the PCs to try and stop it before a breakout or find a cure after one involving them in some complicated politics inside Celene. Primarily my concern is if such a plot as the elves of Celene commiting such acts would stand up.
