    What map is this?

The only map of Greyhawk I've seen at Maldin's site is the Tetreault map from Living Greyhawk Journal. Between that map, the map from Ruins of Greyhawk and the maps from the City of Greyhawk boxed set, I thought I'd pretty much seen every map of Greyhawk and NONE of them have EVERY street named. My research only gave the name Brick Lane as the only street within that general area with a confirmed name, so the rest I admittedly made up.

If you have a map that shows all the street names, you have something I didn't even know existed, and that I certainly don't have access to. Sorry. :(
