The pronlem being, I don't specifically remember:
I think there was a user on the board who mapped the entire of the Western Oerth - and beyond - in very close detail. I mean the entirety of the Chainmail lands, with names, and a small gazetteer. Is this the one you're looking for?
If so, you can find a larger version here and the blown-up versions of each regional (Darlene-sized) map on my blog here (look for the "Mapping beyond the Flanaess" posts). I only did a gazetteer for Zindia/Woguo (aka Nippon), but keep meaning to do one for the other regions as well...

-I just noticed something. Is it just me, or do individual realms get larger outside of the Flanaess? That's not necessarily a bad thing, but it does mean that much of Oerik is a lot more centralized than the Flanaess. Maybe that's why the adventurers are there...
