Placing the classic modules

As a relative Greyhawk neophyte, I'm having a hard time pinpointing exactly where some of the old iconic adventures take place, particularly because I only owned a smattering of them and therefore am drawing most of my information on wiki write-ups of each module. Some are very helpful: The Lost Caverns of Tsojcanth, for example, states the entrance is in the Yatil Mountains (which is also the setting of the follow-up Forgotten Temple of Tharizdun). So while I might not have an exact location, I know I'm at least in the right ballpark. Conversely, others like The Sinister Secret of Saltmarsh are really vague and seem like they could be dropped anywhere, though I'm guessing some of you grognards have deduced and supported very specific locations for it.
Then things start to get really flustering when I look at classic adventures like Keep on the Borderlands which seems to be alternately "claimed" by both Greyhawk and Mystara/Known World as belonging to each setting.
Over in the AD&D 1st Edition subforum I found a thread called "modules" wherein Cebrion stated:

Unfortunately the hyperlink is broken, as that sounds *EXACTLY* like what I'm looking for. Does that module level/location chart exist somewhere else on this site?
