Can someone please tell me how many issues of the LGJ were published and where?

I know the initial 18 or so "issues" were included with Dragon magazine, at which point they switched over to Dungeon to at least issue 25.

Beyond that I don't have any information.

There's an index here.

The first five issues were published independently of any other magazine; they arrived in the mail with an RPGA membership.

The last issue was #24.
