Is there already an underwater city adventure that would work under Nyr Dyv? The only one I know of is X7 War Rafts of Kron. That is set in sea water but that isn't a big deal.
I saw a reference to the City in the Lake of Unknown Depths in the Outdoor Geomorphs playing aid and it got me thinking.
I found references to the Isles of Woe and see that some people don't actually set the islands under the water.
Maybe I should forget about X7 and think about the lake as a portal to the lake in Mordor to explain where the OD&D Hobbits and Orcs come from.

From my recollection:
The Isles of Woe were originally islands ruled over by powerful wizard-priests (Flan?). I believe Tzunk(?) used the Codex of the Infinite Planes and caused a catastrophe which caused the Isles to sink. They still exist far below the surface of the Nyr Dyv.
Additionally, I believe I got that information from one of the Oerth Journals. It describes the Greyhawk Dragon, Wanyrocathistis (the Worm of Woe), who is cursed and resides in the sunken Isles still. Living Greyhawk featured an adventure or several that took place in the Isles of Woe, as well.
There are more published underwater adventures than I thought. Our own Canonfire! Chronicles features Agnosco Adventum, which you may be able to convert from Jeklea Bay to the Nyr Dyv without much trouble.

“Maze of the Morkoth” in DUNGEON #70 features a ruined undersea city. “No Stone Unturned” in DUNGEON #72 features partially flooded ruins beneath a surface city. |
